Mahopac Central School District has partnered with a a new athletic registration platform called FinalForms. Registration for Fall 2024 sports will begin on July 22nd for High School Sports and August 9 for Modified - Grades 7 and 8 sports.

Please be on the lookout in your email regarding this new platform and registration information.

IMPORTANT - PLEASE READ

As of 8/1/2023 All physical exams must now be on the Required New York State Health Examination Form.

  • Please note that "Family Cardiac History Reviewed" must be noted on the physical.

  • Sports Participation - The Required NYS Health Exam Form needs to be dated within 12 months of the start date of each sport season.
